Advance registration is required for all vaccines. Visit lmu.edu/wellbeing to register, and email HRWellbeing@lmu.edu to confirm that your insurance is accepted.
Please bring your Anthem insurance card to your appointment.
Please note: This clinic is for non-Kaiser Permanente members only. Kaiser Permanente members are encouraged to visit their nearest Kaiser medical office for a walk-in appointment.
