Flu shots will be available for faculty and staff during the Benefits Fair on Thursday, Oct. 16, from noon to 2 p.m. in the Burns Lounge. Registration is required.
Kaiser Permanente members
- Flu shots will be subsidized by Human Resources Wellbeing
- COVID boosters will not be available for KP members
Anthem members
- Both flu shots and COVID boosters will be available
Important: A completed consent for vaccination form—Vaccine Administration Record (VAR) is required. Please print, complete, and bring it, along with your insurance card, to your appointment.
If you are not covered by Anthem, Blue Shield of CA, Aetna, or Cigna, please email a photo of your insurance card and date of birth to [email protected] to confirm eligibility.
